In plain language
There is a category of moment where insight is useless. Two in the morning, heart going, the whole day rearranging itself into evidence that you have made a serious mistake. Nothing you understand about yourself is reachable in that state, because the part of your brain that holds the understanding is temporarily offline. What is reachable is your body and your hands.
That is what DBT skills are for. They are not a philosophy of your life. They are a set of concrete moves for the twenty minutes when the wave is cresting, plus a longer set for the conversations you keep avoiding because you cannot predict how they will go. I use them as part of individual therapy, and I want to be plain about what that is and is not before you book anything.
Dialectical behavior therapy was built around a dialectic, which is a fancy word for holding two true things at once: you are doing the best you can, and you need to do things differently. Both. Not one softening the other. That stance turns out to be enormously useful for people who oscillate between excusing themselves and flogging themselves, which is most of the anxious high performers I see.
The skills come in four groups. Distress tolerance is what you use when the emotion is already at nine and the goal is only to get through the next half hour without making it worse. Emotion regulation is the slower work of reducing how often you arrive at nine. Interpersonal effectiveness is scripts and structure for asking for something, saying no, and keeping the relationship intact while you do. Mindfulness runs through all of it as the ability to notice where you are before you react.
Two examples of what this actually looks like. TIPP is a distress tolerance sequence that works on the body directly, most usefully a temperature change: cold water on the face changes your heart rate within a minute, which buys you the gap that reasoning could not. DEAR MAN is a structure for the conversation you have rehearsed forty times and never had: describe, express, assert, reinforce, then stay mindful, appear confident, negotiate. It sounds mechanical written down. In a real conversation it mostly stops you from either exploding or evaporating.

What this actually involves
Step through it. This is the shape of the work, not a script we follow line by line.
Where it starts, how fast it climbs, what the last decent exit point was. Most people can only see the top of the wave. We find the two earlier moments where a skill would still have worked, because a skill at nine is much harder than a skill at four.

- This is skills-informed individual therapy, not a comprehensive DBT program: there is no weekly skills group, no phone coaching between sessions, and no consultation team, so if you have been referred to full-model DBT you need a program that offers it
- If you need crisis or inpatient-level care, weekly outpatient therapy is not the right level of care and I will help you find what is

How I Use It
Where this sits in the rest of the work
Skills come first in almost every course of therapy I run, whatever we are ultimately working on. If your Tuesday is unsurvivable, examining your childhood is not the priority yet. Once the week has some floor under it, the rest becomes possible, and often people notice that the deeper material surfaces on its own once they are not spending all their capacity on staying upright.
They also stabilize trauma work specifically. Before EMDR processing, I want you to have at least one reliable way to bring yourself down, because processing asks your nervous system to go near something difficult and come back. The skill is the coming back. In couples work, the interpersonal effectiveness pieces get used constantly, usually alongside Gottman structure, because knowing what you want to say does not help if the delivery starts a different fight.

Is this full DBT?
No. Comprehensive DBT includes individual therapy, a weekly skills group, phone coaching between sessions and a therapist consultation team. I offer skills-informed individual therapy. If you need the full model, I will say so and help you look for a program.
Do I need a diagnosis to benefit from DBT skills?
No. The skills were developed in a clinical context but they are useful for anyone whose emotions outrun their thinking, which includes a lot of otherwise very capable people.
What is TIPP?
A distress tolerance sequence that changes your physiology quickly when an emotion is at its peak. The piece most people find easiest is a temperature change, for instance cold water on the face, which slows the heart rate enough to make thought available again.
Will I get worksheets?
Sometimes, in a stripped-down form. I would rather you have two skills you actually use than a folder you never open, so we adapt the standard handouts to your life and your phone.
Can we do this over telehealth?
Yes, and rehearsal works fine on video. Telehealth is available Monday through Friday anywhere in California, and in person in Bankers Hill on a limited basis.
